What is a GP Management Plan (GPMP)?
A GP Management Plan is a written care plan created by your GP and practice nurse in consultation with you. It outlines the treatment, tasks, and goals for managing a chronic medical condition.
What Happens When a GPMP is Organised?
Together with your GP and nurse, you will:
- Identify your health priorities
- List actions to help manage your condition
- Determine what other health and community services you may need
- Set outcomes you’d like to achieve from your care plan
Who Can Benefit from a GPMP?
Anyone with a chronic medical condition expected to last six (6) months or more, including:
- Diabetes
- Asthma
- Arthritis
- Cancer
- Heart Disease
- Osteoporosis
- Depression and/or Mental Health Conditions

What is a Team Care Arrangement (TCA)?
If your GP determines that input from other health professionals is beneficial, a Team Care Arrangement (TCA) will be prepared.
With your consent, your GP or practice nurse will contact relevant Allied Health Professionals to join your care team.
Once confirmed, your doctor can complete an EPC referral, giving eligible patients five (5) Medicare-subsidised visits to allied health providers each calendar year. These visits can be used with one provider or split across several.
What is an Allied Health Professional?
Allied health professionals assist with physical or mental health through diagnosis, treatment, or rehabilitation. These may include:
- Aboriginal Health Workers
- Physiotherapists
- Dietitians
- Podiatrists
- Audiologists
- Diabetes Educators
- Mental Health Workers
- Occupational Therapists
- Pharmacists
- Exercise Physiologists
- Speech Pathologists
- Psychologists
- Chiropractors
What is a GPMP/TCA Review?
The review ensures:
Your goals are on track
Adjustments are made if needed
Continued coordination with your care team

- Ask your GP for a copy of your GPMP and/or TCA
- Book an appointment with your chosen allied health provider
- Return in 4–6 months for a GPMP/TCA Review
- Continue following your care plan recommendations
- Book another review after 6 months to discuss your ongoing progress

How long does a care plan take?
Around 45–60 minutes, with your GP and nurse working together to prepare your plan.
How many visits can I have with an allied health provider?
With a valid GPMP and TCA, you’re eligible for 5 subsidised visits per calendar year.
Who is eligible?
Your GP will assess if you meet Medicare’s eligibility requirements for a care plan.
Will there be any costs to see an allied health professional?
Some allied health providers may charge a gap on top of the Medicare rebate. Please check directly with your provider.
Will there be any costs for the care plan?
No. GPMP and TCA plans are bulk billed—no cost to you.
