At Port Sorell Medical Centre, we’re committed to helping you stay on top of your health.
With your consent, we’ll include you in our clinical reminder system — a service designed to keep you informed about important health checks and preventative care appointments.
How You'll Receive Reminders
Reminders are sent to you via:
SMS
Letter (postal mail)
This allows you to easily book appointments and stay on track with your care.
What You’ll Be Reminded About
You may receive reminders for:
- Chronic disease management plans
- Cervical and breast cancer screening
- Immunisations
- Other preventative health needs
National Screening Program Participation
Our practice also works closely with State and Federal health registers, including:
- BreastScreen Tasmania
- National Bowel Cancer Screening Program
- National Cervical Screening Program
You may receive additional reminders from these organisations about screenings relevant to you.

Participation is voluntary. If you’d like to opt out:
